    Based on a Blank 8 1/2 x 11 Two part pre-collated laser cut sheet set. Part 1 is produced from White 22# Laser CB (We will print

    alignment marks for label placement). Part 2 is Blank Canary 21# CF. One 4 x 1/2 Latex / Perm / Piggyback label applied to the face of part

    one. The label has a vertical face slit to create two smaller sections. Both parts of the form and both sections of the label will have a matching

    barcode w/ a human read number // We cannot guarantee no missing numbers // Bulk packed in polybagged cartons // No Glue

    I haven't used Ward Kraft, but I know they do odd things like this too.
