we had a very inefficient one when I started my job 2 1/2 years ago. When we purchased our canon 10,000, part of the package was G7 certification and required a good operating procedure. So we started writing down step by step how we would use everything in the shop no matter how simple the tool (literally have a procedure for how to answer the phone, voice mail script and how to hang up the phone), so that literally anyone could come in and be able to pick up and go with some for of success. As new people would come in, they would have to go through the step by step procedures and make sure they made sense and worked as written (sometimes there would be software/firmware updates that moved things around). We also have a daily list of things that we do/need to do to keep things moving smoothly. As it stands right now, the procedure binder is in a 2 1/2" binder (which does not include the canon manual as that's around 800 pages)
